OUR SERVICES

Home Bound Healthcare Hospice services are delivered by a team of highly skilled professionals dedicated to provide quality patient care and family support. Unlike many other agencies, Home Bound Healthcare Hospice is able to offer continuous care to clients.

Home Bound Healthcare Hospice has designated an interdisciplinary team which, in consultation with the patient’s attending physician, prepares a written plan of care for each patient.

When a cure is no longer possible, Home Bound Healthcare Hospice recognizes that a peaceful and comfortable death is an essential goal and that intensive palliative care focusing on pain relief, comfort and enhanced quality of life within the framework of resident choice,

is a desired goal for the terminally ill.

THE MISSION OF HOME BOUND HEALTHCARE HOSPICE IS DRIVEN BY THE CONCEPT THAT THE DYING PROCESS IS AN INTEGRAL PART OF THE LIFE CYCLE THAT SIGNIFICANTLY IMPACTS NOT ONLY THE PATIENT, BUT ALL MEMBERS OF THE PATIENT’S FAMILY AND FRIENDS.

  • MEDICAL SIGNS THAT YOUR LOVED ONE MAY NEED HOSPICE CARE

    • Weight loss / Poor nutritional intake
    • Progressive decline in status despite curative treatment
    • Frequent hospitalization or ER visits
    • Recurrent symptoms of heart failure at rest
    • Repeat or multiple infections
    • Increased or uncontrolled pain
    • Progressive or profound weakness and fatigue
    • Shortness of breath with or without oxygen
    • Changes in mental status
    • Unable to speak or communicate meaningfully
    • Increase in dependence on ADLs (Activities of Daily Living)
    • Not seeking / Discontinuing dialysis
