Home Bound Healthcare Hospice is a hospice agency that was founded with love by two Registered Nurses in 2010. Home Bound Healthcare Hospice successfully completed their licensure survey in 2011 with no
deficiencies, and accredited by the Accreditation Commission for Health Care (ACHC).
Home Bound Healthcare Hospice team has the experience and advanced training to go beyond the clinical aspect of end-of-life care and creatively work side-by-side with each client and the ones they love. They are committed to the belief that each end of life journey is unique and interweave that philosophy into every patient’s course of care. The Home Bound Healthcare Hospice team is dedicated to always listen carefully to each client and work endlessly to support their vision.
Volunteers are an integral part of the Home Bound Healthcare Hospice team, filling roles that range from direct patient contact to providing clerical and administrative support for the organization. Our Hospice volunteers describe their work as personally gratifying, intellectually stimulating, and emotionally meaningful. <Eva has worked as a Registered Nurse for 13 years. Since joining the Home Bound Healthcare team in August of 2010 as an Agency Supervisor for the Sycamore location, Eva has served patients by providing quality nursing care to our skilled home health patients. Prior to joining Home Bound Healthcare, Eva has worked at North Shore University Health Care Systems – Skokie Hospital as a Registered Nurse Clinician/Charge Nurse in the Emergency Room since 2003. Eva has provided critical care nursing, direct patient care, and served as a charge nurse and preceptor/educator for new staff. Eva has also worked as an Ortho/Surgical Nurse for Illinois Bone and Joint Institute since 1999, working closely with and providing direct assistance to surgeons. In total, Eva has nearly 25 years of patient care experience, practicing as a Licensed Practical Nurse since 1991.
> Doug Zelden has been a non-denominational chaplain working in hospice care for about 10 years. He has been working in pastoral care for over 30 years, since he was first ordained as a Rabbi. He also has advanced degrees in education and theology. He has been with Home Bound for almost 4 years as chaplain and Director of Senior and Community Services.
>Jocelyn is a Licensed Advance Practice Nurse and Registered Professional Nurse.
<Neliza joined the Home Bound Healthcare team in 2008 as an Administrative Nursing Coordinator for Home Health then Hospice where she was later promoted to Assistant Administrator. Prior to that, she was an Admissions Director and Public Aid Coordinator for a Skilled Nursing Facility. She is also a CNA who is committed and dedicated to her work. Her goal is to provide the best quality of service to the patient and the family and to ensure that the hospice team is well educated regarding hospice and compassion towards patient’s end of life. Neliza has a Bachelor of Science degree in Computer Science and Associate degree in Business Computer System.
Denise Beaufait, MSW
Medical Social Worker / Volunteer & Bereavement Coordinator
<Denise is the Medical Social Worker for our Hospice Team. She also manages the Bereavement services and Volunteer coordination efforts for the Hospice office.
